#444 Utilize database rather than plaintext flat files when utilizing SCEP one-time PIN

Bugzilla Bug #621339 - SCEP one-time PIN can be used an unlimited number of
times
addressed this issue through the creation of plaintext flat files.

It would be desirable to utilize a database for storage of this information
rather than the plaintext flat files.

This covers user friendly infrastructure for CA's SCEP including new pin and entity maintenance, authentication, and etc.
