#2342 Hardcoded database name in vlv.ldif for TPS

Currently VLV indexes for TPS are always created in userRoot database:

dn: cn=tus-listTokens-vlv,cn=userRoot,cn=ldbm database,cn=plugins,cn=config

dn: cn=tus-listActivities-vlv,cn=userRoot,cn=ldbm database,cn=plugins,cn=config

The userRoot needs be replaced with {database} which will be replaced by the pki_ds_database parameter during installation.

A (manual) migration step should be provided to fix the indexes in existing TPS installations.
