#2300 TPS CS.cfg tokendb.allowedTransitions need instructions for supported transitions

TPS CS.cfg tokendb.allowedTransitions need instructions for supported
transitions.

Steps to Reproduce:

Due to changes in CS 9.1 code for tokendb.allowedTransition, CS.cfg needs
update for the allowed/not allowed transitions. 0:4, the uninitialized ->
active transition is not supported since it bypasses the enrollment operation.

"tokendb._073=# TEMP_LOST_PERM_LOST = 5," should be removed from CS.cfg since 5
is not a supported transition.

CS.cfg can also contain instruction to have allowedTransition only if the
transition label exist for the same. Creating new allowedTransition and a
transition label as per instructions in
 http://pki.fedoraproject.org/wiki/Customization#Customizing_TPS_Token_State_La
bels may not work without changing the code.

Per CS/DS meeting of 04/25/2016:
