#1370 pkispawn: installation with HSM from external CA should hold off prepending token name in serverCertNick.conf till phase 2

It appears that when installing with HSM from external CA, pkispawn prepends token name into serverCertNick.conf too early which causes server not able to start after phase 1. It needs to hold it off till phase 2 when the real server cert has been generated.
