#6366 Extend CA ACL tests for test cases with CSR containing Subject Alt Name

Provide test coverage to several use cases related to #6291.

The test cases should test for SAN enforcement as well as enforcement of an relationships between services and hosts such as 'managed by' in conjunction with CA ACLs.
